Number Portability Forms

You may download the forms below that will be needed to start the porting process.

"Letter of Authorization for LNP" allows PhoneOverNet to transfer (port) your number from your current phone company.  This form will be required for both business and residential porting.

"Request for Release of Customer Information" authorizes your current phone company to release information to us.  This form will be required for both business and residential porting.

"Toll-Free Number Porting Request" is only needed if you are wishing to port an existing toll-free number to us.

After completing the necessary forms, either scan them and email back to support@v-pointnetworks.com or fax them to 704-817-5066.
